Remote Patient Monitoring (RPM)

  • We virtually monitor a patient’s health as it relates to the following diseases: Heart failure, COPD, Hypertension, Heart Disease, weight management, Type 2 diabetes.
  • The result – a specialized care plan tailored to an individual’s needs.

Transitional Care Management (TCM)

  • Ensures patients who have been recently discharged from hospitals or other facilities have access to primary care doctors and specialists who can help deliver the healthiest outcomes.
  • By partnering with JoinCareTeam, a TCM program can be available to individuals in need based on the health system’s unique workflow.

Remote Therapeutic Monitoring (RTM)

  • Designed to help patients and providers more effectively manage data tied to the use of medical devices.
  • Medication Therapy Management with a JoinCareTeam care manager working with local community pharmacies can make sure prescription dosage and usage are accurate.
