EquityRxTM aims to provide a 360°-care approach with care management and primary care telehealth services to enhance the health outcomes of underserved populations. Working alongside community pharmacies, this program seeks to improve access to care, advance health equity, and increase program adherence by offering personalized and preventative care.

Priority Services
| 1. | Annual Wellness Visit (AWV)
A yearly preventive care appointment to create or update a personalized prevention plan based on current health and risk factors. It aims to catch potential health risks early to prevent diseases and disabilities. |
| 2. | Remote Therapeutic Monitoring (RTM)
Focuses on tracking patients’ adherence to prescribed medication regimens, monitoring medication responses, and managing side effects in real time using digital health tools for better therapeutic outcomes. |
| 3. | Social Determinants of Health (SDOH) Assessment
An assessment that collects non-medical data about the patient, such as their living situation, access to foods, employment status, and transportation options, to better understand their social needs and how to support them. |

Realizing the 360° Patient Care Approach with Multiple Services like RPM + FIT Tests
Currently, around 52,000 people die in the U.S. each year due to colorectal cancer, while an estimated 691,000 annual deaths are attributed to hypertension. According to research from the National Quality Forum (NQF), improving two quality measures – colorectal cancer screening and blood pressure control – could save approximately 144,000 lives over ten years.
